• Ideal for those of you who have a high deductible/underinsured/uninsured
• You must be our patient to benefit from these prices.

HOW

• Please submit the form on the left
• We can advise on which labs you need if you aren't sure
• If you would like to know how much something is that isn't listed on our price menu, just ask. 

PAYMENT

• We will build a requisition and request your approval of the final price  prior to charging your credit card.
• You will receive an itemized receipt. 
• If you don't have a credit-card on file, please email us this form
• Don't forget you can use HSA funds for this.

SCHEDULE

• Schedule your draw appointment at www.labcorp.com or in the Labcorp patient app
• Choose a PSC (patient service center), they will receive an electronic requisition from us, less error. 
• Take your paper copy of requisition to be on the safe side.
• We suggest you take the receipt with you when you get drawn to ensure that Labcorp staff don't bill you.
